The claim form the insurance company requires you to complete and submit varies by insurance company and by the type of coverage.

For students covered under a student accident and sickness insurance policy, please click on the link below entitled: College & University Student Health Insurance Claim Forms Only. Then select the school you attend from the drop-down list and click on the link for claim form.

For all other claim forms, please type in your school or organization's Policy Number then select Go and the applicable claim form will load.
